- Be homebound because it is extremely difficult for you to leave home without the assistance of somebody
- Require skilled nursing or therapy services on an intermittent basis but on specified frequency, and
- Obtain the doctor’s order confirming that you are homebound and need intermittent skilled care according to an appropriate plan of care.
